Over the past couple of years Ive found myself engaged in projects with global, multi-billion dollar medical device corporations, leading teaching hospitals, information technology leaders, investor-driven start-ups, and regional hospital systems. It has delivered an interesting vantage point to witness the foreshadowing healthcare reform has donned across the landscape. While many of the changes are still yet to come, the impetus of change is already rippling through the sector.

Having the opportunity to see things unfold from both sides of the medical device/hospital, producer/consumer relationship does not go unappreciated. It is a privileged overlook that reveals not only the drivers behind a rapidly changing marketplace, but the initiatives both sides of this symbiotic relationship are scrambling to implement.

While we all trust that surgeons and physicians are guided and steeped in the science of medicine, we must also appreciate that once theyre in practice medicine continues to evolve scientifically and technologically.

Medical science does not stand still. The nearly $30 billion in annual, research tax dollars invested by the National Institutes of Health is pushing the envelope forward at an ever-accelerating pace. And this figure doesnt include the private research and development dollars of biopharmaceutical and medical device companies. In fact, it has recently been projected that humankinds scientific knowledge is doubling every five years. How many peer-reviewed, clinical studies can we expect even the finest surgeons and medical practitioners to read every year? Surgeons and physicians do the best they can to keep pace with the state of the science, but traditionally, they practice medicine within a relatively small community of peers, institutions, and colleagues. The practice of clinical medicine, unlike many other technologically-driven industries, has been fragmented. All of the critical-to-quality variables that have been captured, analyzed, and applied in aeronautics, electronics, software, automotive engineering, pharmaceutical manufacturing, even the frying of the perfect fast-food french fry have been unattainable in clinical medicine. Until now.

The federal requirements for automating patient records is unleashing a new age in medicine. And patient records are just the beginning. The application of informatics, the ability to capture large, statistically significant pools of clinical data regarding demographics, genetics, presenting conditions, treatments, surgical procedures, peri-operative care, and patient outcomes is driving a seismic shift in how medicine will be practiced in the future. It is probably the last frontier, the last industry to benefit from transposing raw data into cogent information that will dramatically effect quality, costs, and the standardization of processes to optimize both of these factors simultaneously.

While Andy Griffith is currently appearing in television ads explaining Medicare changes to seniors, and the White House is praising its upcoming health care overhaul, the facts of how Medicare will change still remain a bit fuzzy.

According to the Wall Street Journal, dozens of Medicare Advantage providers plan to cut back vision, dental and prescription benefits. Some plans are eliminating free teeth cleanings and gym memberships, and raising fees for hearing aids, eye glasses and emergency-room visits.

Medicare Advantage plans will take the biggest hit when the health care overhaul starts to take effect next month, mainly because Medicare Advantage plans are privately run plans that offer additional benefits beyond traditional Medicare. Obamas health care overhaul cuts to Medicare Advantage will open up the doors for 30 million Americans who currently dont have health insurance c overage. By taking some funding away from Medicare Advantage, money can be put towards those 30 million uninsured.

Democrats say the payment cuts are fair because Medicare overpays private insurers to run the plans. The government now pays private insurance companies an average of 9% more to operate the plans than it costs the government to run traditional Medicare, according to the Medicare Payment Advisory Commission, an independent congressional agency. That allows insurers to offer richer benefits to enrollees. (Wall Street Journal Online)

As for standard Medicare plans, they will not change, a common misconception among seniors according to Time.com. In a July poll, 50% of seniors believed health care reform would cut benefits that were previously provided to all people on Medicare, and that Medicare patients will have to spend more out of their own pocket. The reality is that while Medicare Advantage will change dramatically, standard Medicare will not, according to Time.com.

The law requires Medicare to pay 100% of preventive care, which includes checkups. The law will also gradually close the Medicare prescription drug gap known as the doughnut hole.

In my San Antonio, Texas based Massage Therapy and Bodywork practice I often treat pain from various syndromes including Piriformis Syndrome. Like many painful conditions Piriformis Syndrome responds quite well to massage therapy and bodywork.

What is Piriformis Syndrome?

Piriformis Syndrome is the name of a painful nerve entrapment condition in which the sciatic nerve is placed under abnormal pressure by the piriformis muscle. The piriformis muscle attaches to the sacrum bone of the low back to the top of greater trochanter of the femur (thigh bone) and lies on top of the sciatic nerve. When the piriformis muscle becomes tight and enlarged, pain results from myofascial trigger points (knots) in the muscle, pressure on the sciatic nerve, and sometimes from the S-I Joint in the low back. Pain from Piriformis Syndrome is usually found in the extreme low back, buttocks, hip, and often the back of the thigh. Most of the time the symptoms are on one side, but may affect both sides in some cases. Additionally, people with Piriformis Syndrome often have trouble sitting for long periods and the pain may worsen when walking or squatting.

What is the cause of Piriformis Syndrome?

There are a number of causes for Piriformis Syndrome including direct blunt trauma to the buttocks, prolonged sitting while working or driving, Morton’s foot, or S-I Joint instability. Frequent or prolonged running on canted surfaces and participation in sports requiring quick lateral movement such as tennis, skiing, and basketball may also overstress the piriformis muscle and lead to Piriformis Syndrome.

Massage Therapy and Bodywork for Piriformis Syndrome

There are many ways that massage therapy and bodywork can be used to treat Piriformis Syndrome. The massage therapist will employ a number of techniques to relieve the condition. For example the massage therapist may use Neuromuscular Therapy or Trigger Point Therapy to eliminate myofascial trigger points in the piriformis muscle. The massage therapist could also use Myofascial Release techniques or Deep Tissue Massage techniques to release adhesions between the piriformis muscle and surrounding tissues while also using Muscle Energy Techniques to gently lengthen the piriformis muscle. Piriformis Syndrome also responds well to positional release therapies such as Ortho-Bionomy which work with the body’s nervous system to release muscles and improve joint movement in the area. Comprehensive treatment with massage and bodywork, in conjunction with a home program of stretching and exercise, usually helps to completely resolve Piriformis Syndrome.
